Re: PPE Order Instructions, To be completed by 7pm ET on Monday, April 13

In an effort to assist with the PPE shortage, Mass Senior Care Association is partnering with the MIT Covid Policy Alliance to place a bulk PPE order for N95 Masks, gowns and gloves from China. We are fortunate to offer MSCA members the ability to order PPE scheduled to arrive in the U.S. on April 20th. If you are interested in participating in this bulk order, we need you to place an order no later than 7pm TODAY, Monday, April 13.
We recognize the immediacy of this decision and ask that you:
First, enter the information requested in this Excel spreadsheet which will calculate your estimated PPE demand for the next two months; and
Second, once you have determined your PPE needs based on the calculator, please access the order form here. Provide the required information (i.e. your facility name, point of contact, quantity needed), and submit.
By submitting this form, you are committing to reimbursing Mass Senior Care Association for the total amount of your order which must be paid prior to delivery of the PPE.
